Hello, I'm Ana Greer, MPH(c), Founder and Principal Consultant of Heartfelt Support & Seal LLC.
As an Authorized OPWDD Support Broker and public health professional, I am passionate about helping individuals with developmental disabilities, their families, and community partners confidently navigate the Self-Direction journey.
I founded Heartfelt Support & Seal LLC because I recognized a need for clear guidance, compassionate support, and organized systems that empower people—not overwhelm them. Families often face complex processes, unfamiliar terminology, and countless decisions. My mission is to simplify that journey while ensuring every individual remains at the center of every plan.

The OPWDD Front Door is the official entry point for individuals with developmental disabilities to access New York State funding and support services. Your journey begins by connecting with your local Developmental Disabilities Regional Office (DDRO).
To start, call the official New York State OPWDD Information Line at 1-866-946-9733. Request a transfer to your local regional office by stating the county you reside in.

Accessing services follows a multi-stage process handled by the DDRO front door team:
Initial Contact & Information Session: Watch the mandatory educational Front Door videos or register for a live virtual session.
Establish Eligibility: Submit a formal clinical packet to the DDRO to prove a qualifying developmental disability.
Select a Care Coordination Organization (CCO): Partner with a CCO care manager who helps gather documents and process your tracking paperwork.
Needs Assessment: Complete the mandatory state assessment to outline strengths and support needs.
Service Authorization: Submit a final service request plan to the DDRO for state budget approva

To establish official New York State eligibility, you must prepare and submit the following documentation:
A Psychological Evaluation including a full-scale IQ score and adaptive behavior testing (must be less than 3 years old).
A Social History or Psychosocial Evaluation detailing developmental milestones (must be less than 1 year old).
A General Medical Report or physical exam overview signed by a doctor (must be less than 1 year old).
